With the parent / guardian’s signature, permission is given to Networking For Life to provide “in-school student achievement support services” to the above mentioned student on a “not to interfere basis at the student’s school during school hours.”
Parent/guardian agrees to pay monthly services fee of $25.00 via website at www.networkingforlife.org by clinking on the Student insurance link not later than 3 days prior to the beginning of each month.
